Complaint Registration Form
The Complaint Registration Form applies to all people involved with the activities of RDA, including staff, volunteers and members. Any person (complainant) may report a complaint about a person, people or organisation bound by the Complaints Management and Resolution Policy and Procedures.
In the first instance we advise that you try and resolve the complaint with the person that you have the issue with. However, we understand that sometimes this is not possible. For further information, please refer to our Grievance Procedure and Complaints Management and Resolution Policy and Procedure.
All complaints will be recorded in the relevant Centre's Complaint Register and/or the RDA Tasmania Complaint Register. All complaints will maintain the privacy and confidentiality of all persons involved. If you need assistance with completing this form, please contact the State Office or our Member Protection Information Officer (contact details available here).
Your complaint will be forwarded to the relevant people to assess and investigate your complaint. You may be contacted to provide more information. You should receive acknowledgement of receipt of your complaint within 3 business days. Depending on the complexity of the complaint it may take up to 30 business days to receive a response with either an outcome or to inform you if more time is needed for a resolution.
